Continuum of Care

I converted the Continuum of Care iOS app for McCann Complete Medical to Windows Phone and Android using their respective native SDKs.

I had access to the iOS source and completed an exact clone in the time frame required. I believe the project is under review across all platforms to be updated in 2015.
